Crusade Trips Documents
 
APPLICATION Focus International Ministrties Trip Application.  An application and other documents will be mailed to you upon contacting us with your desire to be a team member of a crusade trip. 
 
MEDICAL & CONSENT FORMS  These forms will be sent to you upon request to be a Focus International Team Member. NOTE:  The page must be notarized for you to be accepted on the trip. NOTE: We also will need a copy of the yellow adult immunization record card.
 
  
PASTORS RECOMMENDATION FORM  This form will be mailed to you with the application and other required forms.  Your pastor should fill out this form and mail back to us.  The address is enclosed on the form.  This form may not apply to those we know through a Word Of Life Wichita Campus or other means.  You will be notified if one is needed.
 
 
PASSPORT  We need a color copy of your passport picture page in the event your passport is lost or stolen. Please make sure when you scan it that the picture is clear and all text is legible.  If you have not yet applied for a passport you need to do so immediately.  We must receive this no later than the ticket purchase date.
 
 
VISA    Kenya, and many other countries require a visa for entry.  In Kenya this is obtained at the Nairobi Kenya airport.  You must also have your yellow immunization card with you that shows you have received the yellow fever and other recommended shots. You will be notified if a visa is required for the country you are a team member of, and the requirements associated with it.
